Confidential New Client Forms

Before you join me in my office for our first meeting, I would appreciate it if you would fill out the forms below. Please link to them by clicking on the name of each form, then go online and fill each of them out. For your privacy, the completed forms will be encrypted and securely transmitted via HTTPS (SSL encryption) to me before your appointment. 

The first is a Client Information Form. Mostly contact information and other basic information.

The second is a Client Disclosure Statement that I must have you read and sign to legally practice Homeopathy in California.

The third is a Measure Yourself Medical Outcome Profile, which is a short form that enables both of us to measure the progress of your or your child's treatment goals.
